2018 JET PROGRAMME APPLICATIONS / SOP THREAD

So I'm in a bit of an unusual situation with my JET application - I'd love some thoughts about what you think the best option would be.

I was accepted into the 2016 JET program as an ALT but had to decline a position (prior to location placement) due to the untimely death of an immediate family member. The aftermath of that family emergency has finally subsided and I am able to reapply for the 2018 JET program.

However, when reviewing my application from 2016 I discovered that I had said I had no history of mental illness when in reality about a decade ago I was treated for about 9 months for anxiety and depression. I haven't had any issues with anxiety or depression since. I truly did not recall this until having a conversation with my doctor this month, who is also a family friend, when he reminded me that during high school I had undergone treatment.

So my dilemma: Do I report the same answer as on my 2016 application because as a previously accepted candidate who turned down a position my prior application will surely be scrutinized - even though this is (now) knowingly lying.

Or do I report the truth and risk having my application disqualified due to discrepancies between 2016 and 2018.

I'm racking my brain over this. I know the truth seems like the obvious choice, but I have been working toward reapplying for this program for literally two years and the thought of being disqualified due to discrepancies between this application and the 2016 app seems so terrible.
