3 out of 5 toadlets dead in 1 week! What the hell am I doing wrong?

Yeah that's why I was shocked when I saw mold. I only put the tank together a week ago. I've used the dishwasher to clean basically all cage furniture for my frogs, snakes, and turtles. If that's toxic how should I clean things?

I have two flightless fruit fly culture jars. I keep one in the tank to provide continuous food, and one to alternate out.

I don't know the exact temperature. I never misted the cage That room is always hot due to my massive snake cage and I live somewhere very humid, plus have aquariums. I guess I thought that would be enough but, typing that out made me realize how dumb that was.

Water, dechlorinated tap. Coco coir, I thought it was a reduced risk of gut impaction and more sanitary than soil.

I do keep an adult of the same species under the same conditions with no issues.
