Accountant made a big mistake on my tax return, what do I do?

You are right, I did sign the return. There was confusion. When I sent through the contract work - he replied he was working on the tax return for the year before and would add my contract work when he got to the current year. So when he sent me my tax return and I didn't see the extra money in there I assume it was the year before tax return he had just done (however, it was the current year return). It wasn't until my sibling pointed out to me that those months I worked were supposed to be in current tax return. I only earnt 13k plus the 8k last year due to covid, so maybe own 1.5k in tax but I am very scared I will get a mark against my name or even fined.
