AITA for not giving my kidney to my half/sister?

Because you are downplaying the risks and discomfort.

I'm presenting them accurately, not downplaying them. Risks are low likelihood but impact could be significant up to and including death.

OP isn’t an asshole for prioritizing his own long-term health.

OP is an asshole for prioritizing their own long-term health at the expense of someone else's life, not for prioritizing his own health.

She can be on dialysis while a properly vetted kidney is found.

She may. OP may be incorrectly reporting the facts. But based on the "If no donation, she dies" scenario presented by OP then this is 100% YTA.

You seem to be extremely upset with OP exercising his rights to medical freedom

If it seems that way to you then I can only surmise that you are projecting too much of your own feelings onto this situation.

Are you on a living donor list by any chance?

I cannot think of any reason where answering that would serve any constructive purpose.
