i am worried i will mess up my med school interview due to my extreme anxiety

Just write down detailed answers beforehand like with specific examples of where you have done things like shown teamwork, communication skills. Make sure you have an answer for most of the questions they could ask you about and write a lot of examples of your experience. Look up example answers online and model your own answers around them.

Then learn all your answers. Get in some practice interviews beforehand.

Also you are allowed to stop and pause before you say something in an interview. You are allowed to stop and think. If you don't understand a question you can say could you explain what you mean. Or if something really stumps you, you can say could we come back to it.

Most important part of an interview is often coming across like an open, respectful and confident person. Nerves are the thing which can change a good interview from a bad one.
