Can someone help me make a work out routine?

Here is what I started out with. Combine strength training (weights) with cardio (running/swimming).

Strength Training (Sunday, Tuesday, Thursday, Saturday)
*Barbell Bench Press 3 reps x 3 sets
*Barbell Squats 3 reps x 3 sets
*Barbell Deadlifts 3 reps x 3 sets -starting weights should depend on your comfort levels. I started with using a 45 kg. barbell bar weighted with two 25 kg. plates on each side. I added 10 kg. as soon as I was hitting a plateau)

*Push ups x 10 or failure
*Pull ups x 5 or failure
*Planks x 30 seconds or failure
*Jumping Jacks x 50 or failure
*Farmer's Walk x 5 minutes
*Skullcrushers x 5 reps x 5 sets using a 35 kg. dumbbell

Cardio
Running on Sunday, Tuesday, Thursday, Saturday
Swimming on Monday, Wednesday, and Friday
You can start your running routine on a 30 minute basis. 1 hour if you have the time. I started slow, doing 5 minute intervals of *running and 5 minutes of speed walking. I upped it to 10 minute intervals afterwards. You should be able to cover 1 - 2 km. in 30 minutes even as a beginner.
**With swimming, Just start with maybe 2 - 3 laps of pure front crawl style.

As you can see, you don't need a lot of fancy exercises. I started my strength training with only the holy trinity of weight training exercises: barbell bench press, deadlifts, and squats. That's all you need. Those exercises alone, if done on free weights, should be able to work all your major muscle groups, as long as you're doing it correctly. Don't forget that proper posture and body mechanics helps a lot too. I only started integrating other exercises into my routine when I got bored and felt comfortable enough to mix it up a bit. It helps if you find a nursing or medical textbook. Study the Anatomy and Physiology of the Muscular System. Understand how each muscle works. Working them out at the gym is basically going for exaggerated motions of what they normally do.

Diet is important too. Make sure that you start your workout with at least 300 calories, whether it be half a protein bar, or a banana with a small black coffee. Finish your workout right away with another 300 calories of protein and carbs. A small chocolate milk usually does the trick for me. You will have to REALLY WORK at changing your lifestyle. Fitness is a marathon, not a sprint.
