That's because even the Heritage Foundation acknowledged the simple truth that demand for healthcare is not elastic. Demand for healthcare does not go up with lower prices, or go down with higher prices. It is solely governed by sickness rates. Consequently, traditional mechanisms that drive markets towards efficiency do not work in healthcare. And on top of that, there are tertiary issues like natural monopolies due to rural infrastructure barriers. Ultimately, even the right-wing loons back then understood the fact that healthcare could not possibly be a purely private enterprise, and that we absolutely needed government intervention to ensure that it remains affordable and accessible to every income class of the society. That realization led to the MA healthcare bill as a compromise between the private and public spheres.
