I genuinely wonder how they decide who gets the longer forms or not.

I recieved the short form so there was nothing related to health but we would have been a good addition to it if it genuinely plays a part in getting support for those at risk.

I personally suffer from 5 different mental illnesses, I am Autistic, have APD and am currently diagnosed with roughly 15 physical health issues (like autoimmune diseases) as well if that plays a part.

My son is also Autistic with his share of mental health and neurological disorders and my parter has physical and mental health related disabilities as well.

I'm not sure what kinds of questions are asked in the long form but we could have provided a ton of information I'm sure.
