Special Power of Attorney (if applicable) Valid Military ID Card Leave and Earnings Statement (LES) Leave/PCS orders Supporting documentation such as copies of estimates for vehicle and HVAC repairs, billing statements for household expenses, and income statements from all sources (including Spouse, if applicable). For any assistance with vehicle repair, a copy of a current driver’s license, proof of insurance, and valid registration for the particular vehicle is required.

Also it depends on how your installation runs it, I can speak from my experience with Fort Carson ACS - AER, 4 years ago, who handed me an application that wanted my bank statements for the past 24 months, because I didn't have $1500 for a new clutch, and when I said no thank you, they wouldn't give me the forms back, and contacted my unit, which black balled me and labeled me as a financial fuck up.
