So this is what happened why you told a patient not to get pregnant and they did anyway? I can see why they refused, as most people don't think a doctor has the right to give that sort of order to them, but merely has the right to warn them of the risks.

That's the most bothersome thing you've posted. That you think that doctors need to give someone a clear go ahead on whether to get pregnant. That getting pregnant is up to you, not them.
If you want patients to actually follow your recommendations, I suggest you change your attitude.
