You shouldn't be thinking "have I lost an engine?"

If you have indications immediately perform the engine failure immediately after takeoff procedures. Have the copilot analyze/confirm. They'll catch it. Or once you're safely established in the climb profile or at a safe altitude check yourself. Catch your dufus mistake if you didn't when you took . Learn a lesson.

What I don't know is if he had had full R trim in, done part of the engine failure immediately after t/o checklist and feathered the R engine creating even more of a R turning desire if it would be recoverable. A failed engine with the other at max already creates a need for close to full rudder. If you combine the failed engine (or self inducing via the checklist) while having full trim in the wrong direction even if you kick in full opposite you're at a buttload in the direction of the trim and "failed" engine.
