O: Attitude: BAR Temp 102.2 Hydration/mm: WNL, pink, slightly tacky gums, CRT < 2 sec Eyes: WNL, no obvious globe abnormalities Ears: WNL externally/visible vertical canal Teeth: nsf Heart: Tachycardia - consistent finding on exam Respiratory: WNL, panting, no crackles/wheezes etc Gastrointestinal: non - painful, no obvious large masses, rectal not performed, multiple bouts of diarrhea in/at the clinic, orange in color Urogenital: No obvious abnormalities Integumentary: WNL, no obvious concerning masses, no fleas/flea dirt seen Musculoskeletal system: WNL, no obvious lameness, no obvious pain on palpation of back/neck, no ROM neck concerns, Page 1 of 2

Neurological: Rear CP's mostly normal at this point - possibly slightly slow, but markedly improved from previous External lymph nodes: WNL Test/Procedures: Radiographs - Vertebral Heart Score 9.1, no pulmonary edema. possible minor pleural effusion present along ventral aspect of lung field vs variation of normal. Moderate gas in stomach and colon, mild gas in large intestines and small. No obvious obstructive pattern or foreign material present. 1 liter subq fluids Cerenia - 3.9ml subq


A: Diarrhea Tachycardia Improving neurological signs Hyporexia Vomiting/nausea


P: Discussion: The xrays show a normal sized heart with no fluid accumulation in the lungs that would suggest any sort of heart disease. I can't say 100% without a full work up by a cardiologist but its very unlikely Beau has heart disease based on what tests I have at my disposal. His neurological signs have improved significantly so far - his feet are responding mostly normal based on todays exam and his gait seems mostly normal as well. We are currently battling his GI upset and I'm hoping this is simply just a side effect of all the stress of vet visits and stress of being given meds along with side effects of meds themselves (namely the carprofen and steroids switch over). I hope that with some supportive care we get him back on track soon. We did fluids under the skin since he is losing fluids in the vomit and diarrhea. Started with a cerenia injection for nausea/vomiting - it lasts 24 hours. Start the cerenia pills tomorrow. Start the Metronidazole pills today for diarrhea as well as the propectalin oral paste. A bland diet (we sent a few cans) should help as well. Please reduce the prednisone to 1 and 1/2 pills once a day since I've documented his CP's are improved and he may be having GI side effects from these meds in part. His breathing heavy, restlessness etc hopefully are just due to the GI upset if you consider how it would feel to have a large amount of inflammation and vomiting/diarrhea happening. Not typically a dog that gets into trash/chews items up etc so hopefully we can avoid that sort of concern. I'm obviously more than happy to continue to help with adjustments, however we've done almost all the work up I have available to me here. So, I hope that these adjustments and some time will get him heading in the right direction. Stay on omeprazole for roughly 6 more days. Finish the sucralfate out Continue gabapentin for pain control - GI upset is not likely to be a side effect of this med, usually just sedation We recommend being on flea/tick and heartworm prevention year-round.
