Yes, he should be on medication. However, the main problem with paranoid schizophrenia is that it cannot be 100% controlled with therapy or medication. It is also incurable. It is typical that paranoid schizophrenics can "function normally" when they are on meds and live a normal life. However, the medication is not 100% effective always so there will be periods when he will slip back into paranoia despite of the medication.

I know someone who dated a schizophrenic for a while and schizophrenic people from my community. They receive very high quality treatment, but schizophrenia always affects their life to some extent. They sometimes still have episodes of paranoia. Despite of treatment many are also unable to work. Some can work and study normally, but there has been cases when a schizophrenic has ended up losing his/her job due to a period of paranoia.

The problem is that if this man has a tendency to be violent (all schizophrenics do not have a tendency to be violent) he could be hell of a problem when he will some day slip back into a period of paranoia.
