He had to be asleep, it just completely fucking unaware if a bot fly out eggs on his lip. That's not common. It's sincerely mostly anywhere away from your face.

It's gross and removal sucks. You basically suffocate the larva, who's ass is occasionally shitting drops of crap out the exit. It causes them to surface, and removal hurts but it's unsurprisingly rewarding when that shit is done. In this case, they're using a liquid to cut off oxygen, which causes the larva to surface more, looking for air. Most people just cover it with whatever for an hour or two and have the same result.

It's gross, it's weird, but money is on this dude either being totally black out trashed when he got hit, or was asleep at the wrong place when it happened.
