I gave a 12 year old the wrong covid vaccine

I had a lady and her husband both come into the room, both to get a shingrix and a pfizer covid. The lady was up first, and for each of them they were each to get one vaccine in one arm and another vaccine in the other arm. I administered the pfizer covid, then had her spin around in the chair to present the other should, and gave her another pfizer covid! Before the syringe was out of her arm I knew I screwed up.. they were both super cool, apologized for being so chatty and distracting, etc. Can't believe I did that. I'm not a new grad at all - one of the most known and recognized managers in the metro area, but hell, that doesn't make me a whole lot less prone to the occasional error.

We all make mistakes... it's super rare and we beat ourselves up, but care of the patient and owning up to our error is the most important. Our companies expect that errors will happen... good job being genuine, caring, reporting, etc. You are in good company.
