Homebrew DMs of Reddit, please share with me the name of your world and a blurb about its lore.

The world is shaped like a Möebius strip.

  • Next to the continent the campaign is set on (the 'Edgelands') is the literal Edge of the World. Waterfalls cascade into the void. Jump off of it, and you'll fall to the other side of the world.
  • A month's travel away by ship, the Great Fountain blasts a sheer geyser into the sky from that edge; that's the way to get back from that other side, if you survive the trip.
  • Legends say you can travel along the Edge of the World, and eventually, you'll end up right back where you started. A few have supposedly done it, but the trip took decades.
    • Gameplay implication: This is a truly fantastic place, so players shouldn't expect some watered-down Euro-fantasy. My adventures are far more 'gonzo' and the group goes with it.

Portals to other planes are naturally-occurring throughout the land.

  • The heart of an active volcano is a doorway to the Elemental Plane of Fire, Dive into the reflection of a full moon in a pool in a virgin forest, and when you surface, you'll be in the Feywild. You just have to be trained what to look for by the right person, creature, or secret society.
    • Gameplay implication: Planar travel can be done by parties without spellcasters as part of adventure design.
  • Some regions of the Underdark have weak barriers between the Prime and the Abyss. Sometimes, powerful creatures break through into our world and start corrupting it.
    • Gameplay implication: It explains how such abominations can exist in relative proximity to civilized lands.

There's a bit more, but that should paint a good picture.
