How to get a high CTR with low CPC?

Same thought here as well... I work in-house for a drug rehab center. They have three centers in three different very competitive states and I do all their PPC advertising, but it's really difficult (I came from a non-lead gen background prior so I've had a hard time adjusting). I see the same domains advertising at first ad spot, consistently for words that are even $100+, I don't know how they can afford to run the ads and make it profitable. I barely make my projections each month and I feel like luck has kept me alive so far... For example, to rank in the top three for the obvious searches someone would make to find rehab treatment like "best drug rehabs" or "best drug rehab near me", is at least $65... My budget is only $1,500 a day for each center, so that's giving me about 23 clicks per day, for each center based on the $65 CPC... but then on top of that they need to have in-network, non-government provided insurance which disqualifies a lot of people from becoming an acquisition and eats the budget up... I tried to drop my bids to rank lower and see if more click volume would lead to more daily leads, but that actually ended up being worse; I assume this is because most people click the top ads, if they have a good insurance plan they've already been accepted to a cometitor and if they weren't they went to the next in position ad to see if they qualify because theothers have already turned them away... then there is also the fact that a lot of people are calling for someone who is an addict and doesnt even wanna go to rehab which makes the phone call go on for an hour to no result or conversion... drug rehab is very negatively stigmatized so you're dealing with very avoidant and reluctant people usually which makes it difficult to convert.

So I have to hope that of the 20-25 clicks I may get on my campaign, they not only do they have qualifying insurance, but also have the true desire and support begin rehab.

The cost of these keywords keep rising and when the call-only ads came out I thought I finally had a way of getting all clicks to turn into phone calls, but it turns out that the CPC's for call only ads were putting me up into $100+ for a call, which made my performance even worse...

By the way, if anyone here has any tips, strategies, or solutions to my problem please let me know or send me a message if you don't want to publicly share... I really like the place I work for now and have good benefits, but I really feel like I'm faced with an uphill battle that has no real solution to it.
