How is not wanting to drink alcohol seen in the USA?

I've noticed people respond kind of similarly to when I decline to eat crappy food or at least a copious amount of food during celebratory events. A lot of people equate celebrating anything with eating a lot and or drinking, and I don't mostly for overall health reasons. When people observe that I'm eating a salad or not drinking during a night out with co-workers for a minor celebration or whatever, they sometimes seem concerned on my behalf like I don't know how to let loose or enjoy myself properly, or even a bit uncomfortable like I'm uptight and cramping the good time. Granted, I do celebrate with food and drinking, but usually for the stereotypical eating and drinking holidays rather than celebrating anything period. For the most part though people respect the choice, I just thought I'd provide a bit more nuance to the discussion.
