if they stuck a counselor on the bridge of the Enterprise-D because therapists were 'chic' in the 80s, what kind of character can we expect in 2017 ?

Though many transgender individuals would certainly undergo such a procedure, there are also a lot of people who are transgender and don't want all the surgery and stuff, even if they could get it easily in the future. I've known some who really enjoy their body and enjoy being a "woman with a penis" or a "man with a vagina" and think that sexual organs and gender are very separate things.

I think people like that would certainly still be around in the future. Someone born with conventionally "male" organs who identifies as a woman mentally and emotionally, dresses as a woman, acts as a woman, but keeps their biology as it is. Spare maybe a few changes like body hair and voice, as most hormone treatments affect currently.

Perhaps in the future, the concepts of what it is to be a man or a woman won't be so black and white. You won't have to be "passing" to be taken seriously as a woman. You wouldn't have to make that full leap physically unless you really wanted to. There will be a lot more grey area in terms of gender identity, so that everyone can find where they are most comfortable on that spectrum.

Transgender individuals wouldn't have to make a full physical transformation in order to be considered "truly" male or female by their peers. They could if they wanted to, but they wouldn't have to. They wouldn't have to convince anyone of who and what they are.

It reminds me of the questions surrounding Picard's lack of hair.

During an interview about the casting of Jean-Luc Picard, Gene Roddenberry was asked by a reporter, "Surely they would have cured baldness by the 24th century?"

Roddenberry answered, "In the 24th century, they wouldn't care."
