HPI: construct a focused narrative that leads us to your diagnosis or at least your differential but try to have a diagnosis in mind. End with pertinent ROS.

PMHx/Surgical Hx: start with the most relevant medical history

Home meds: Start with the most relevant medicines and group by organ system or diagnosis of possible. This provides any remaining listeners some grounding.

Allergies: include reactions. If it's long can simply state do and include only if relevant to plan.

Family Hx: try to stay relevant and unless this is peds or directly relates to the plan I don't need to hear about non first degree relatives

Social Hx

ROS: I hate hearing boiler plate phrases for billing in a presentation. Please don't say "all other ROS reviewed and negative except as noted in HPI". For presentations I recommend transitioning to a SOAP note since most patients are admitted in the evening and presented in the morning you could start here with "the patient slept well overnight and had no new complaints this morning". This is a style choice. I only suggest that student goes with what works here and do it the same way every time in the future.

EKG: have a copy in hand

Imaging

Assessment/plan: short summary followed by a problem list starting with the most relevant problems. When listing a differential begin with the most common diagnosis and try to hold back rare or unlikely diagnoses until the end. Third year students should consider simply getting to this past uninterrupted a success.
