A letter to my GP about him and I

I suggest a formal meeting with those involved in my care, of which you, Dr. NoName, are my Primary. It seems that just one provider, or providers working alone, will not bring resolution to my issues. In 2016, is this too much to ask from the health care system? Has it gotten to that point?

Unless you are seeing a bunch of people that compromise a "care team" within an overarching organization this is not something that is practical or ever really happens. If people are working for different practices/organizations the logistics of this would be impossible. I'm a NP in primary care and it is difficult to get a specialist even on the phone most of the time, usually we communicate through notes.

This is a good "get it out of your system" type note but a bad one to send to a healthcare provider. I'd recommend you do not send this to a healthcare provider.

If you're not happy with your PCP I recommend finding a new PCP, but there is no sense in you burning bridges which could make your life more difficult in the future.
