I think your argument is weakened by taking shitty, irrelevant shots at groups that are already suffering. I do want people with emphysema and other smoking related issues to get treatment. I do think that people who struggle with addiction, even if they are actively using, should get medical care. Longterm diets have a low success rate in morbidly obese people, and bariatric surgery has a much higher success rate in medical research when compared to lifestyle change alone. Bariatric surgery statistically will often extend peoples' lives, improve their quality of life, and reduce future medical costs, so I'm glad people are able to access it with Medicaid. None of these things are frivolous or a waste of money, because I don't want people to suffer and die without at least attempting to give them time to make progress.

There is no competition where you have to prove that these groups (who are already really stigmatized and underfunded by society) are worse than trans people. Creating these caricatures of people who you think are less deserving of medical care "versus" the productive trans people isn't a good argument. It isn't progressive to make the same tired joke about how you think substance use disorder is actually people being stupid and selfish instead of understanding it as a complex mental health issue. I want everyone to receive health care that is scientifically proven and based on medical advice from their doctors. It wouldn't surprise me if state Republicans did want to cut funding for obesity and substance abuse with arguments just like you're using. Republicans don't care about people in poverty and they don't care about medical research.
