Menstruation makes me dysphoric; for a game where you create your own character, I'd probably prefer to have an option to turn it off. (And in the game. Sorry.) This is just a personal preference, though. But for a game with a preset protagonist, I think it could be an interesting element to include.

On the other hand, most games don't really stimulate hunger, thirst, or waste excretion, so menstruation seems like an odd bodily function to single out. It could be interesting as an added dimension to a survival game, though. Or maybe a novelty RPG: Period Quest! Brave the perils of the personal health aisle! Master your chosen birth control! Choose your weapon - pad, tampon, or cup? Someone made a bra shopping RPG, I feel like this could legitimately be a good topic for a game.
