My night in the Peds ED

meanwhile my night in the Peds ED was like

-ICARE / broviac / TPN / GT / fever / draw labs / hang abx / dispo HOLD for 5 hours because house is full

-status-epilepticus / 50% RA on presentation / bagged / iv-access / ativan / NRB 15L/min / CHTnoncon / dispo HOLD for 3 hours because house is full

-neonate / fever / ROS-workup / 3x failed straight-cath attempts by previous RN / mom furious / LP-neg / nationwide shortage of cefotaxime / have to wait over an hour to get order changed to & pharm to send gent

-ICARE / broviac / TPN / GT / "fever" T38 on arrival / waffled with Resident & ICARE team for 2 hours over whether or not to do full ROS-workup / repeat temp 37.7 / ended up finally pulled the trigger on labs / bolus / abx / CXR / cath UA/UC / peripheral Cx / admit ICARE / dispo HOLD because house is full

-neonate / inguinal abscess / US backed up 6x patients / finally got mine up to US / ? lymph node vs isolated abscess / waffed over IV-access/labs/MIVF for 2 hours between ED and Surg teams / orders went in 5min before change of shift / I did the workup so I didn't fuck my relief with a hot-mess

-recurrent periorbital cellulitis s/p 4x OR-I&D procedures & PICC-placed for home abx / worsening swelling & pain / NPO for possible OR / labs/abx/ivf / dispo HOLD because house is full

did I mention NEDOCS-5? Charge: "Hey when are you going to get (Room#) upstairs?" Me: "Uh when I get to it?"

I walked out of there an hour after I was scheduled to leave and felt this weird lingering sensation in my asshole like someone with a telephone pole for an arm had tried to use me as a puppet.

But I did have a "fever-no-fever" with "tactile temps" (Me: What was the temp at home? Mom: Well I didn't take a temp but he FELT warm / afebrile on presentation / bounceback 3rd ED-presentation in one week for URI) that we did a CXR on and caught what we thought to be early-onset PNA (or at least atelectasis). So there's that.
