Usually one of your parents would complete an Appendix 1 when you do your student loan application. But in your case, you would apply for student loans without using your parental information and submitting an Appendix 1. You are requesting to be classified as a modified independent status student. It is IMPOSSIBLE for your parent(s)/step-parent/sponsor/legal guardian to complete Appendix 1.
You can continue with your application; however, your application cannot be processed until the following documentation is submitted:
- A modified independent status appeal request form, available on your application timeline; AND
- A notarized statement detailing the nature of the family rift, how long the rift has existed, and why it continues; AND
- A letter from a professional third party (not a member of your immediate or extended family) on business letterhead, testifying as to the truthfulness of your explanation and providing an independent assessment of the situation. This letter must state how long the person has known you and in what capacity. The third party could be a counselor, psychologist, religious advisor, doctor, teacher, social worker or law enforcement officer; AND
