They start back where they left off, which is their previous bottom. "Bottom" for lots of people is the realization that they are killing themselves and will die soon. So a not unusual wagon-faller-offer will show up and look sane, healthy, normal. Very quickly, like say three stiff drinks, he's been shut off, thrown out for outrageous behavior, the cops have been called because he refuses to leave, he's fought with them and ended up arrested and banned from the premises. The next day he appears, refused entry, and within days/weeks he's banned from every bar in the area, hanging out with the hard core drinkers (all banned from everywhere) behind the liquor store drinking the cheapest nips and taking whatever drugs come their way. Some people have much higher bottoms, so they probably stop showing up for work or whatever, but the ones who die, this is the pattern.
