I replied to support saying absolutely nothing happened and that I can provide dashcam footage.

This is very good. You have denied it and have proof. Preserve that proof in several different locations on different media. Save it to multiple USB sticks, keep one at your parent's house, upload a copy to the Cloud, etc. Do not leave it in one place where you may lose it.

They replied and just asked me to fill out the incident form.

Never do this. The only thing you can do is weaken your position if you give them information. Speak to a lawyer before you do anything else.

Stop doing things like this immediately. You are making it much easier for a personal injury lawsuit to proceed against you. They will name you, Uber, and anyone else relevant in the lawsuit. If insurance declines to provide you with a lawyer, you will be paying for one out of pocket. That is $275/hour. Not a good idea.

What should I expect moving forward?

If you are lucky, you haven't given anyone information that can be used to sue you. Lawyers always hope they get to their clients before they talk to the police, opposition in civil lawsuit, etc. If they don't, it makes their lives much harder having to fight against the opposing side and their own client's statements.

tl;dr If you are asked to fill out paperwork and someone is trying to claim personal injury against you, talk to a lawyer. Do NOT just fill out the paperwork blindly. You may be handing over hundreds of thousands of dollars of your future earnings voluntarily when you do.
