premed advice needed

I really want to go to a t20 medical school

Med school is my dream.

Do you want a fancy name on a piece of paper or do you want to be a physician? If the main reason you want to pursue t20 is because of vanity, I would highly advise you to reconsider your priorities. A doctor is a doctor regardless of whether they went to Harvard or the middle of nowhere.

I am so scared I am behind or doing cookie-cutter things and will not stand out.

You need to reframe this perspective. Instead of trying to do things that you think "stand out", do things that are meaningful to you.

Frankly, you aren't going to do anything that medical schools haven't seen before. Adcoms have seen every single accomplishment, from Olympic medalists to Navy Seals to published researchers. There is essentially nothing that will be "omg" to adcoms.

Instead, what they do care about is that you find issues, topics, communities that you are passionate about and that you pursue those interests. Perhaps you're interested in pediatrics. Go work with underserved pediatric populations, volunteer at daycares, do research in clinical pediatric care. That will help shape your application and help adcoms understand who you are.
