1) When are your symptoms the worst? Morning, afternoon, or night? Morning. 2) Have you ever smoked, done drugs, drank alcohol? If so how much? done some hard drugs during my early twenties 3) In your lifetime, have you been exposed to chemical compounds or fumes? Have you willingly ingested or inhaled chemicals or fumes? No. 4) Are you an IBS-D or IBS-C sufferer? Do your symptoms vary between the two? Both , but mainly C 5) During a flare up is your pain in the stomach, upper intestine, or lower intestine? Large intestine 6) What foods do you suspect to be triggers? How often do you consume these foods? under cooked or raw , certain vegetables and water , clean pure drinking water. 7) Do you participate in stressful events, careers, or live in a stressful environment? Yes my work , yes my neighbours are durg addicts 8) Does anything relieve your symptoms and how long does it take to work/how long is it effective for? sometimes smoking tobacco will relieve it , I also take sulfasalazine 9) Do you have a history of disease or illness? Are you or were you once anemic? I am Anemic and no history of illness 10) How is your dental hygiene? Do you find that you have dental issues regardless of your dental care? Dental hygiene is great 11) Did your symptoms begin at the onset of puberty? Yes when I was 15 12) Do your symptoms wake you from sleep or begin upon waking up and starting your morning routine? They begin when I wake and start my morning routine 13) Do you exercise regularly? If so, what exercises do you do and how many times per week? I walk every morning , run every third day and wight lift 3 times a week. 14) Women: if you have children, did your symptoms increase or decrease during pregnancy/after pregnancy? 15) Are you prone to infections? no 16) What is your nationality/which culture do you identify with? White anglo saxon 17) Have you been in any traumatic accidents? no 18) Are there foods that do not seem to affect you some days but do affect you other days? What foods and how? Pizza , can some times give me D, but other times it won't same with mcdonalds. 19) Do you participate in any activities that relieve your IBS symptoms? I don't know of any 20) How frequent do you experience your IBS symptoms? It fluctuates sometimes it can be between 5-10 times a day others and can be none at all. 21) Have you been evaluated by a medical gastrointerologist and what tests have they performed on you? What were the results? Yes , was told to take sulfasalazine and eat dark leafy green vegetables and take iron tablets.
