I really don’t think you would have to disclose that. I don’t really think that counts as probation or discipline? A corrective action plan is a step-by-step plan that’s put in place to address specific shortcomings or errors in the operation of your business.

Worst case, and I don’t recommend this for a genuine C + F issue, you can always talk to the dean at your school after admittance and if they want you to, you can submit an updated addendum. In preparing students for the bar exam, deans do talk to the student body about how the bar’s C+F requirements is more exhaustive than the schools are and to update to include anything at all that the bar might care about.
