Is still used as a joke, people despite feeling uneasy about still make it into a joke and laugh about it. This is the same. A small dick can be both an insult and a joke. If when someone mentions “roast beef lips” and you are hurt by that then maybe you need to come to terms with the fact that you have “roast beef lips” and take it as a joke and not a blatant attack at you which it wasn’t. If you’re insulted by it then it is your insecurities that are the problem and not the joke itself. A small dick can be a joke, a racist joke can be a joke, it all depends on how accepting you are.
