Russian trolls 'spreading discord' over vaccine safety online

It's not only foreign states doing it, but pretty much every news station. I don't really like to use the phrase "fake news" as it has been misused by Trump to refer to things he doesn't like, but the reality is that their is massive bias in news and language is abused to make opinions, rather than just present the news. It's especially awful when authors start talking about statistics, oftentimes they literally reinterpret the results because they haven't understood the paper and that reaches the masses who willingly consume that wrong information.

You see things like the lawsuit against Monsanto/Bayer, where the verdict wasn't based on science, or maybe it was, at least in the opinion of these people who might've been subject to wrong information. Green parties across the world spread their opinion and pseudoscience about GMO and nuclear energy, right parties theirs on climate change and other things. You can literally see the division of ideology across news organizations where you can classify them into left and right. That's not only a phenomenon that happens in the US but also in Europe.
