[Serious] What is the worst thing you've ever seen happen that you could have stopped?

In middle school there was this kid in my class who we shall call Andy. Now Andy was a pretty normal socially awkward, introverted, super quite 7th grader who liked yu-gi-oh. Although despite all this, he was not picked on by anyone because he was around 5'8, second tallest of all the other kids and there was easier targets around. So Andy had a few other friends who he played yu-gi-oh with at lunch, his friends weren't so lucky with the bullies and other kids. Andy never really tried to stand up for them as he and his friends were outnumbered and was also pretty scrawny. Now I rode the bus with Andy and he was always early to it and rode in the front. Until one day when there was a student who was required to sit in the front because he had a nose bleed, and had a friend with her so they could not share a seat. But they only arrived until later when the bus was full, and there were only seats in the middle. This was where a group of bullies sat. So relentless bullying followed. He kept his head in his lap and backpack the whole ride home. I sat a couple seats behind them and overheard one of them whisper to another about getting off at his stop to "have some fun with him". I could have told the bus driver what they were planning, but had the common fear of being a snitch and then being their target, as I was pretty low on the school hierarchy. So they all got off at his stop and i watched as the group of assholes followed Andy closely until they got out of site of the bus and road. Two days later Andy showed up at school with bruises on his arms and a slight limp and a large bandaid on his left cheek. He never confessed and the bus driver was not paying attention. I didn't want to tell the administration because again, I felt like shit about not telling the first time and did not want to get involved. I still think about how easy it would have been to stop all of it.
