Closing Bank info/relationship: Chase Open/Pending accounts at time of shutdown: • Chase Amazon (06/15), $2k CL • Chase Freedom (02/16), $1.5k CL • Chase Sapphire Preferred (11/16), $5k CL • Chase Freedom Unlimited (03/17), $10k CL • Chase Ink Preferred, BRM (10/17), $5k CL • Chase Ink Cash, BRM (App 01/18, denial one week later with shutdown) Stated reason (if given) for shutdown: • Too many new accounts and too many recent inquiries Was it your bank account(s) or credit card(s) that were shut down - or both? • Credit cards only Have you attempted to get your accounts reinstated? How far have you escalated it? • Multiple attempts to reinstate current shutdown were denied due to prior shutdown and reinstatement. They would not change their minds. CFPB also failed for same reasons (Inq/#Acct) Personal Info: FICO: • 750 TU, 741 EQ, 704 EX AAoA: • 2 years

No Have you deposited money orders into a bank account that you have with the bank who shut you down (do you shit where you eat)? If so, what is your volume per month? • No MO deposits into Chase How do you usually pay your credit card bill? Have you changed the method by which you do so recently? • It varies per month. I mostly use Chase to pay using funds transferred into Chase, but occasionally will use 1-2 outside banks. Ratio or percentage of MS compared to organic: Mostly organic Spending Behavior / 'Consumer Profile': How much organic spend were you putting on cards issued by the bank who shut you down? Few thousand per month Have you ever sold the bank's points to someone else? No Have you filed multiple chargebacks with the bank in the past 12 months? No Has this bank ever taken adverse action against you before? Has any other bank? If so, when? What happened? • Yes. I was shutdown in August and it was re-instated. This was provoked by a $5k bank funding charge that posted and I floated for 3 weeks before I paid it off. I called First Party Fraud, who stated that the shutdown was due to “a large cash advance” (it posted as a purchase, but whatever). Reasons stated in that shutdown were due to too many inquiries and accounts. The initial shutdown was reversed after several days. Have you in recent history significantly increased or decreased either the level of your organic spend or MS? • Spend has been the same, mostly organic with only MS on my one Freedom card. Do you have any new derogatory marks on your credit report? Are you sure? Have you checked since you were shut down? • Yes, no derogatory marks. I check using CK and Experian every month.
