There's no "altruistic sentiment" during residency or fellowship. People make little to nothing during this period of their life and can't wait to get certified and start working as an attending, of course you would've known that as a "doctor". Nobody covers the entire code blues, take care of 70 beds, deal with the code whites, and carry the consult pager in the hospital all alone at night because of "altruistic sentiment". People do it as part of their training and count the days till they're done.
