Store Swapped SIM Without Asking for ID, Name or Account Passcode

I think this should be reported. I think you should find the email contact for EVP of retail Jon Frier. ME's need to be verifying name and ID. (stores do not have access to customer PINs). Hearing stories about the prompts for ID verification just being clicked through is infuriating.

Account security aside, in my store people get in our faces all of the time because they changed a married name on their account and now it doesn't match their ID. Or because their ID expired, or they only have a picture of it on their phone etc. And we turn all of that shit away. People "call corporate to complain" or leave nasty Google reviews because we followed proper procedure. So hearing that there are others out there who probably cave to bad customers is doubly infuriating.
