Read the original post. My reply was concerning the OP's very specific situation, questions, and concerns. It was not an anti-IF post. I've no issue with IFing.

Yes. However, it does not do so through strict Ketosis. It also uses gluconeogenesis through catabolism of it's own muscle cells. At low bodyfat the body is unable to meet all it's needs with bodyfat alone when their is insufficient dietary intake of fat and protein. Particularly if engaging in strenuous activity.

Now specifically addressing the OP issues:

They specifically want to have a certain range of ketones in their blood for specific medical reasons. That might not be possible for a number of reasons that might all be working against them. They have a low bodyfat. Are expending a lot of calories exercising. Are eating in a deficit to lose fat. IFing is not a bad thing. However, for their specific issues it might not be the most conducive practice.

To imply someone cannot survive - and thrive - a full day without eating, unless they look like a skeleton, is completely BS.

Totally not what I said man. Thriving was not mentioned nor was surviving. The subject is how to "be in a certain range of ketones to have therapeutic effect for my med condition" all day long. Ketosis is not required to thrive or survive when fasting. However, the OPer wants to be in it.

1kg of fat is 9000 calories. The female's essential fat is around 4-6kg depending on body size, so any fat in addition to that would make living even a few days without food a non-issue.

She is 110lbs. She might be 20% BF. That gives her 22lbs of BF to pull from. Currently the data says we can only pull about 32 kcal/lb of body fat. Thats 704 kcal spread over the whole day or about 30 kcal an hour. Most of her consumed calories are being used up during her workouts and she is IFing the rest of the day. Her totally sedentary TDEE is 1559. So she is at least using 65 kcal an hour just to exist. If she has used up all consumed food and exhausted the amount of kcal she can get from her fat stores, which is only 30 kcal then where is the rest of the energy coming from. My theory is her body is digesting it's own protein through gluconeogensis in order to glucose to make up the remaining 35 kcal an hour she needs. Hence, dropping her out of ketosis at the 4-5 hour mark.

My theory is that by ingesting a tiny amount of fat at the 4 hour mark she would inhibit that process and remain in ketosis.

Remember her issue is not Ifing. It is that she wants to keep her blood ketones at a certain level to treat a medical condition.
