Is their an items that turns you to an undead?

My DM loved bonkers magical items. Among them I was given a helmet that made me undead, but because of shenanigans I was already undead and by the rule of double negatives it made me "alive" again. I also had an axe that exploded when I missed. The axe built up charges every time I hit up to around 12.

At one point I had the maximum number of charges. I decided to just suicide bomb myself into a dwarf I didn't like because he was talking shit. So I tackled him off the cart we were in, waited for the party to get clear, took off my helmet and buried my axe into the dirt beside his head and shouted ALLAHU ACKBAR!

DM laughs hysterically and tells me to roll an obscene amount of dice. I roll. He describes to the party the fountain of blood and meat that showers the cart.

Party spends some time collecting my pieces and bring me back to the clerics that owed us a favor. They sewed me back together regenerated a few things and put the helmet back on my head. Apparently this revived me as an undead but anytime I took the helmet off I would die.

My DM basically enjoyed the idea that I was now an "effectively" immortal undead fighter with the intelligence of a stump.
