Thoughts on what is causing this failure? My suspicion is insufficient cooling but I don't know how best to fix that. It'

Idk if this would be a proper explanation, but heat transfer typically works where heat travels from the hotter source to the cooler source.

So in this instance, warping can occur (lifting corners), because the top layers are cooling and shrinking faster than the bottom, therefore pulling on the bottom layers causing the lift.

You lay the first layer with a bed temp = 60C and nozzle = 220C. We can say the first layer then will be somewhere between 60C-220C (very roughly).

Jump to maybe layer 4, bed temp on the bottom is 50C, while layer 4 is ~220C. The change from layers 1-4 is 50C - 220C. Heat will want to be drawn downward because of this difference, cooling the top layer more quickly (recall the nozzle fan cooling as well so the top layer not only is losing heat to the bottom layers, but to the ambient air as well).

My best guess is to minimize the change in temperature the best you can buy increasing the bed temp and possibly lowering your nozzle temp a bit. Let me know if this works for you :) I'm still learning too. I had some issues with lift, but not as drastic as yours so my problem subsided when tweaking temps a bit.
