What company has forever lost your business and why?

Sprint, I currently have them as my provider, but come March will not. Here's why: I signed up when they did the "switch and we'll cut your bill in half." I had a $90/month bill with Verizon and so I decided to change. I got the exact same phone and at the store was given a tablet and told it was "included", I said I didn't want it, and they said I had to take it, but could sell it if I wanted. Anyway, the contract they gave me at the store was $45/month for the exact same thing I had with Verizon. Pleased, I walked out of the store. More than a month passes and I never hear anything from them until one day my phone stops working. I go to the store to find my phone has been shut off because I didn't pay my bill, which I never received. I confirmed my address was correct and then found out my bill was $150 (included a reconnect fee, my normal bill was like $120). I was thoroughly pissed off and demanded to know what happened because that's not what I agreed to at all and had the documents to prove it. They informed me that the store had it wrong and gave me the wrong information, the store said that it wasn't their fault and corporate has it wrong. Almost 18 months later, I've never received a bill, can't access my online account and can't get that situation settled.
