What to fix?

Kind of hard to tell with just one pic, but I think your face and jaw are somewhat asymmetrical, though you may be tilting your head a little I'm not really sure. To correct that you could have jaw surgery, i would recommend that if there are functional problems with your bite. If there aren't, a v-line (jaw shaving) surgery could correct the asymmetry and make your jaw smaller.

I would recommend zygoma (cheekbone) reduction on you, you have very prominent high cheekbones and it throws off the balance of your face and makes your face look wide at the top.

Another thing I think would help is eyelid surgery to reduce the epicanthal fold, this would open your eyes up a lot and make them look bigger.
Also the tip of your nose is quite bulbous, a rhinoplasty would solve this.

You don't really need all these surgeries, but this is just my opinion on what would improve your face the most.
