Small town near me recently had a scandal where a female officer mysteriously turned up dead. One year-long investigation later, in this department with less than 20 officers, she had been sleeping with three of them. One officer found out she was "cheating" on him, and killed her. The officer that murdered her was married with three children. Chief of police and several lieutenants/sergeants knew the female officer was sleeping with coworkers, and had turned a blind eye. Their leniency ultimately contributed to her death.

Chief of police, every lieutenant, and every sergeant was fired immediately afterward. Every person above rookie pay-grade gone. My city had to lend them officers to fill the gaps while they hired to fill the openings. Gigantic shitshow.
