Any piano-knowledgeable focal dystonia specialists to recommend in the San Diego area?

I would consider just canceling your appointment with the regular neurologist and schedule one with a neurologist specializing in movement disorders. There are a ton of neurologists (especially in any large metropolitan area) that just do movement disorders (Parkinson’s, dystonia, essential tremor are the big ones).

They tend to be really good at clinical diagnosis.

I have cervical dystonia and even though I hated my first movement specialist for other reasons, he was able to identify that, not only did I have dystonia, but that my neck wanted to rotate to the right—something I never noticed on my own.

If it’s urgent, like you aren’t able to work and you don’t have time to work through a lot of tinkering, I’d highly recommend going to the Mayo Clinic in Rochester, MN. Also the building actually has a piano in it so you could take them down to the piano and demonstrate the problem.
