Doctors office wants to know if I use a seatbelt or own a firearm…

I'm sure this will be an unpopular opinion, but everyone knows way too much about everyone else these days, and I find these questions intrusive. I go to a doctor to be treated for illness or discuss specific health issues. I wouldn't answer questions like this. If I'm suicidal, it's none of their business unless I bring it up and ask for help. Seat belt use doesn't always save lives; sometimes it takes them, and whether or not I use it regularly is up to me. Once something is put in writing, it hangs around forever! It's in a permanent file somewhere on a server. Have people today so totally given up their right to privacy that they'll answer any and all questions asked of them by anyone, without hesitation? And okay, maybe I'm paranoid, but as someone who grew up NOT having to fill out lengthy questionnaires before seeing a doctor (or dentist!), I suspect some of this info is sometimes shared. The fine print often implies or outright confirms it. When they ask for my SS number, I always leave it blank. What possible reason do they need this for except to run a credit check? Hell no. You're a doctor; treat me. It's your damn job. The SS number, which I was told all my life was never to be given to anyone, is now a joke, so many people have access to it. I'm entitled to privacy, and I balk at anyone wanting information about me they have no right to.
