I'm not a college applicant po pero ang mga ginawa ko are:

1. I clicked the downloadable forms (ito yung COE form and Conditional Admission Contract (CAC) form, in my case)
2. Filled out the COE form.
3. Filled out the Conditional Admission Contract (CAC) form (para ito sa mga wala pang complete form 138 or form 137 or transcript of records na maipapass sa UST). *Check mo lang yung box ng hinihingi nila na wala ka sa CAC form. For example kung hinihingi sa sa'yo ay form 137 at wala ka pa nito, check mo yung box ng form 137 and ilagay mo yung date kung kailan mo ito ipapass sa UST. (may nababasa ako dito sa Reddit na nagkaproblem sila sa DATE NA NILAGAY NILA AT SABI SA KANILA NG UST HANGGANG AUGUST 1, 2022 LANG DAW PWEDENG MAGPASS NG FORM 137 OR TRANSCRIPT OF RECORDS.)
4. I completed the requirements na hinihingi sa COE form (yung applicable in my case)
5. After filling out the needed forms and completing the requirements/credentials needed, I submitted them to UST via email address na binigay according to the program that I chose. Pero sa iba ata na college applicants via google form link nila sinubmit credentials/requirements nila.
6. After that, I waited for a confirmation email from UST. Nakalagay sa email na natanggap ko ang date of enrollment ko.
7. Wait for 5-10 working days para mafill out ko yung Personal Data Sheet. May link na nakalagay sa result portal ko at email confirm na natanggap ko.

*Refer to number 4.\* Yung mga requirements na pinass ko together with the COE and CAC forms are:

- PSA birth certificate

-2x2 ID picture

- Certification from the principal's office stating the expected date of graduation [para ito sa mga nagfill out ng CAC form na wala pang Certificate of Diploma/completion at form 138 in my case. (pero sa college applicants form 137 or transcript of records ata ang hinihingi nila, sorry po di ako sure about sa college applicants.)].

-Certificate of Good Moral Character

P.S. may deadline ang pagpapass ng credentials/requirements for confirming enrollment.
