Yes, many trans people do transition and then go on to live happy, normal, healthy lives. Yes, some trans people struggle their entire lives.

An FtM who is also small, asian, and would like to be a large, black Canadian would find themselves in the same predicament as your example... and if there was a potential medical transition- perhaps your example would make an attempt and try it.
