ELI5:Why are zits and acne so hard to solve? It's right on the surface and there's a hole that leads right to the problem. What's the hard part?

Accutane should only be used as a last resort after exhausting all other treatment options, this stuff has the potential to do serious and insane damage to your body. I had a horrific experience with this drug, one that nearly brought me to the point of suicide. I've had severe acne since I was fourteen and by the time I was in my twenties I had had enough of trying numerous different regimens while spending thousands paying out of pocket for medicine like retinoids and antibiotics without ever getting fully clear skin. I begged my derm for this drug and began taking it without realizing how it would interact with an prior corrective surgery I had in order to fix a deviated septum in my nose. The drug dried out the grafts in my nose causing them to warp and begin to disfigure, when I realized this I immediately stopped taking the drug but my nose never returned to it's previous shape, and at the same time I was just beginning the drugs initial breakout phase, which resulted in nearly every single poor on my cheeks to break out. I kid you not every last one. It was so awful that I literally couldn't bare to be seen again in public, and so I dropped out of college and remained in my room for nearly five months without any contact with anybody but my friends online. This was the darkest period in my entire life. Six years later I still have acne on a constant basis, but at this point I feel like I've just excepted that I'll probably be old before my acne finally goes away and have just started to try and live my life instead of patiently waiting for my "acne free" period to begin which I originally had thought would come by twenty-one or so. The point is that to many people this drug seems like a golden ticket to perfect skin, beauty, popularity or whatever it is all those ads they play on MTV try and convince kids comes with good skin, but in reality people should consider exactly how bad their acne really is and if it's worth risking ones health and well being for.
