You would need to see the policy coverages, limitations and exclusions. Most “market place” policies don’t cover it, don’t take my word for it, talk to an agent and request policy coverages and exclusions. Once you have that review, also when purchasing a policy make sure your out of pocket deductible is low, so you pay less. You can also ask for an EOC (Evidence Of Coverage) to make sure it is covered and not excluded. Most of the time for the insurance to cover it, you do need doctor recommendations, and to meet insurance criteria, and follow the process they set. Just because your surgeon is in network does not mean the surgery is covered, you wanna make sure the procedure is covered.
