Looking for a dataset that is more accurate than NPI or CMS

What you are looking for does not exist, but you are free to start building it. To get the most accurate provider info you will want to start with state records, most (all?) states sell these, but it is straightforward and will give you their office. These are not perfect, but you can get them.

As for the insurance and practice affiliation... well, you are in for a long haul. There is no simple way to build this one, and the only accurate one will be going down your list and asking every office which insurance they accept. As for provider affiliation, it depends on exactly what you mean, but this is not as straightforward as it seems and I'm not sure there is a good method to get this. At best you can lookup public sources and see what comes up for each physician in your database, and then maybe cull any that are geographically implausible.

I suggest building a team, hiring a data collection agency to do the calls, and expecting to spend millions doing it (top of the head guess, anywhere from 5-10). Even then, your final result will be shoddy, not to mention outdated upon completion.

States have penalties and beg providers to keep records up to date, and still regularly don't have up to date records.
