Good Evening, I am deeply sorry to hear that and I completely understand my apologies again I was not meaning to come across as impatient.

I guess what I mean is that I answered the ss5 correctly as it asked for my father's name and not name at birth which was specifically called out in the mother's section. As per the SS5 it did not request the name at the father's birth like the mother's section and I correctly answered it by providing my father's legal name it has been all my life and is on my birth certificate. I assume this would be correct per the wording of the ss5. I am probably overthinking this like my wife says
